OPTION 1: FBI Fingerprinting Affidavit
Complete the affidavit only if you have lived in PA for the past 10 years.
OPTION 2: FBI Fingerprinting Criminal History Report: approx $20-30
Fingerprinting is only needed if you have NOT lived in PA for the past 10 years. If you are required to complete this step, you will have to travel to be fingerprinted.

- Drop of Sundays at designated locations (place papers in sealed envelope):
- Fusion – use the mailbox located outside of the Hub doors
- Kid Life – drop off at the check-in counter in the Kid Life lobby

- FINANCIAL REIMBURSEMENT
Volunteers who are required to pay for their FBI Fingerprinting Report will be reimbursed only when the clearances are obtained for the purpose of serving as a volunteer at Living Word. Complete the reimbursement form. Return this form with the other required documents.
SPECIAL NOTE - All background check results and clearance information are kept strictly confidential and will only be accessed by authorized church staff.
- If you already have the above-listed clearances, please submit a copy to the Ministry you serve in. Reimbursements will not be offered for clearances previously obtained for other purposes.
